In order to help you arrive well prepared for your office visit, we have posted necessary office forms here for you to print out and complete prior to your visit. If you are unable to print the forms, simply arrive a few minutes early to your visit; we will have these forms available in the office for you.
Insurance Information
At Headache Control Clinic most insurances are accepted. Please call the main office to check if your insurance can be billed.
We accept all PPO, POS and Medicare insurance plan. We are also part of Gottlieb HMO and Unified Physician HMO group. All HMO patients must obtain an authorization from the primary care physician before make an appointment.
